Access this website and enter the information requested; then download the document and/or print!
VaxViewNM.org enables individuals, parents, and guardians to access, save and/or print official immunization records. This eliminates the need to carry multiple or aged documents and allows you to access your shot record anytime you need. VaxViewNM.org is mobile friendly and uses two-factor authentication. Please see attached flyer! (Click link on the left for IMP-NMSIIS-VaxView-Flyer.pdf) Please ask your child's immunization provider to be sure to enter your correct contact information in to "NMSIIS" when recording the vaccine. If site does not work for you, it is because they do not have this information. If this happens call the NMSIIS Help Desk at 1-833-882-6454.

Hello parents!
I need updated immunization records for 7th graders who have received their Tdap and Meningococcal vaccines . I also need immunization records for new enrolling students who have previously not attended an APS school.
Please feel free to email through ParentVue or directly to me @ Jeanelle.adkins@aps.edu.

Medication at School
If your student has a specific health need such as allergies or asthma, please download and complete the relevant form with your doctor.
If your student would like to keep non-prescription medications for use at school, please complete the OTC medications form. The Health Room does not carry Tylenol, Ibuprofen, or other medications for student use.
